Description

Post-void dribbling can occur after phalloplasty with urethral lengthening when urine remains within the reconstructed urethra after voiding and subsequently leaks from the urethral meatus. Although often benign, this can be distressing for patients and may affect confidence, hygiene, and quality of life.

This educational video demonstrates a simple urethral milking techniquen designed to help patients empty residual urine from the neourethra after passing urine. The technique involves applying gentle pressure along the underside of the phallus, starting proximally and moving distally towards the urethral opening, in order to encourage any retained urine to drain. The manoeuvre should be performed gently and only after voiding is complete.

The video is intended as a practical, patient-facing resource for individuals who experience urinary leakage after voiding following phalloplasty with urethral lengthening. It also provides clinicians with a clear visual explanation that can be used during postoperative counselling.

Urethral milking may help some patients manage post-void dribbling; however, persistent, worsening, painful, or obstructive urinary symptoms should prompt clinical review to exclude urethral stricture, fistula, infection, or other complications.
