Description

Acquired buried penis is a challenging condition that can significantly impair both sexual function and genital aesthetics. This video presents a comprehensive Triple Plasty technique combining monsplasty, suspensory ligament release, and ventral scrotoplasty to optimize penile length and exposure. The procedure began with liposuction of the infrainguinal fat pad to debulk the overlying adipose tissue and improve penile exposure. Following this, dissection of the infrainguinal fat pad was performed, taking meticulous care to identify and preserve the spermatic cords bilaterally. Attention is then turned to the suspensory ligament complex, where both the fundiform and true suspensory ligaments are sequentially released at the pubic symphysis under direct visualization, preserving the dorsal neurovascular bundle. This maneuver facilitates corporal externalization and redefinition of the pubopenile angle. The procedure concludes with penopubic tacking and ventral scrotoplasty to enhance genital contour and functional exposure. Postoperative outcomes include significant improvement in stretched penile length, enhanced projection, and restoration of normal penile angles. The patient was discharged in stable condition, and follow-up demonstrated satisfactory healing and improved genital aesthetics. This video provides detailed anatomical insights and a reproducible technique for surgeons managing patients with acquired buried penis seeking functional and aesthetic penile length enhancement
