Description

This video abstract demonstrates tunical excision and grafting with a bovine pericardial patch for the correction of severe dorsal penile curvature in Peyronie’s disease. The procedure is indicated for advanced cases, typically exceeding 60 degrees of curvature, where plication techniques are insufficient. Following circumferential penile degloving, the dorsal neurovascular bundle is carefully dissected and mobilized. The fibrotic tunical segment is excised, and a tailored bovine pericardial graft is anchored under minimal tension to restore tunical integrity. In the presented case of a 90-degree dorsal curvature, complete straightening was achieved, erectile function was preserved, and no intraoperative or postoperative complications occurred. This technique provides durable anatomical restoration, maintains penile length and function, and highlights meticulous neurovascular management, precise graft sizing, and intraoperative verification as key factors for optimal outcomes.
