Description

Spermatic cord block (SCB) targets afferent input from the genitofemoral, ilioinguinal, and spermatic nerves, and should be considered for patients with chronic scrotal pain (CSP) (AUA Guidelines, 2025). An injection of 8 mL 2.0% lidocaine 1 cm inferior and medial to the pubic tubercle can be both diagnostic and therapeutic. Patients reporting over 50% pain improvement can be offered microscopic denervation (Beamer et al., 2021). Otherwise, blocks can be repeated as needed. If pain persists after four hours, surgical management may be warranted. The therapeutic effects of SCB can be enhanced with a multimodal approach (AUA Guidelines, 2025).
