This video demonstrates the feasibility and safety of performing an extraperitoneal single-port robot-assisted radical prostatectomy with bilateral lymphadenectomy and simultaneous three-piece inflatable penile prosthesis implantation using the da Vinci SP® system. Combining both oncologic and functional procedures in one session allows for radical tumor control while achieving early sexual and urinary recovery, avoiding the risks and costs of a second operation. A 61-year-old patient with high-risk prostate cancer (Gleason 9) underwent this innovative approach with no complications, minimal blood loss, and a rapid recovery. At six months, the patient showed undetectable PSA, full continence, and optimal prosthesis function with excellent satisfaction. This case highlights a promising strategy for selected patients, optimizing recovery and preserving penile length while maximizing functional outcomes.
