This video, through live surgery, illustrates a subcoronal approach to placement of a 3-piece inflatable penile prosthesis with simultaneous circumcision. Multiple surgical approaches can be utilized for the implantation of an inflatable penile prosthesis (IPP), including through penoscrotal and infrapubic incisions. Using a subcoronal approach to place an IPP confers numerous advantages for patients and implanting urologists. It allows for all three IPP components to be implanted via the same subcoronal incision, minimizing the number of incisions required. It also provides superior visibility and access for urologists when exposing the tunical layer for implantation. Furthermore, subcoronal implantation allows for corrective procedures such as circumcision, penile plication, tunical incision/excision grafting, and manual modeling to be performed simultaneously using the same incision. This patient is 71 years old presenting with severe medically refractory erectile dysfunction. We demonstrate a surgical technique for concomitant circumcision and subcoronal placement of a 3-piece IPP.
