Description

This video presents a streamlined, reproducible technique for simple metoidioplasty—a masculinizing genital gender-affirming surgery. Using labia minora–based flaps for ventral shaft coverage, the approach enhances phallic projection and contour while preserving native voiding, erogenous sensation and vaginal function. No intraoperative complications occurred, with minimal blood loss, brief hospitalization, and rapid recovery. Early outcomes demonstrate symmetric phallic contour, preserved sensitivity, and high patient satisfaction.

Simple metoidioplasty remains under-described in the literature; this technique offers a low-morbidity, function-preserving option for transmasculine individuals seeking genital affirmation without urethral reconstruction. Key operative steps, pearls, and postoperative results are illustrated to promote safe and reproducible adoption across surgical centers.
