Scrotal Flaps for Penile Skin Reconstruction

Penile skin defects, which may result from trauma, infection, or iatrogenic urological surgery, often require surgical reconstruction when spontaneous healing fails. Conventional repair strategies including skin grafting and staged scrotal flap procedures are limited by suboptimal cosmetic results, prolonged hospitalization, and increased patient discomfort, while single stage flap reconstruction has emerged as an improved alternative for complex penile wounds. This study presents a case of single stage bilateral scrotal flap reconstruction in a 16 year old male patient with severe abdominal and penile scar contracture secondary to a traffic accident. The patient presented with incomplete penile extension and painful erection due to post traumatic scarring. Single stage pedicled scrotal flap repair was performed to reconstruct the defective penile skin envelope. Postoperative follow up demonstrated satisfactory functional recovery and aesthetic outcomes at both donor and recipient sites. In conclusion, single stage scrotal flap reconstruction, when performed with meticulous surgical technique and appropriate patient selection, is a reliable and effective approach for the repair of extensive penile skin defects. It favorably restores penile anatomical integrity and physiological function, reduces surgical morbidity, and significantly improves patients’ quality of life.
