Description

This case report describes a 39-year-old man with a history of recurrent epididymitis who presented with hematospermia and ongoing genital discomfort. Diagnostic imaging, primarily MRI, revealed a rare epidermoid cyst arising from the right seminal vesicle. After conservative management efforts, including a failed attempt at endoscopic catheterization, the patient underwent definitive treatment with robotic-assisted laparoscopic vesiculectomy. The management of such rare benign lesions is fundamentally driven by symptom presence. In this symptomatic case, a minimally invasive surgical approach provided superior visualization, enabling careful dissection and complete excision while minimizing operative morbidity. Postoperative imaging confirmed successful removal of the lesion without complications. This case underscores that surgical excision, particularly with advanced minimally invasive techniques, offers an effective, curative option resulting in full symptom resolution.
