Description
Inflatable penile prosthesis implantation is the gold standard for refractory erectile dysfunction, but device-related complications may occur. Pump malposition can impair function, reduce patient satisfaction, and require surgical revision. We present a case of a patient with a 26 cm corporal length, managed with a 22 cm IPP and 4 cm extender. Although initial recovery was uneventful, follow-up revealed proximal pump migration toward the penile shaft, causing difficulty with deflation and limited accessibility. Surgical exploration demonstrated coiled tubing and a fibrous capsule encasing the pump. The capsule was excised, tubing mobilized, and the pump repositioned into a dartos pouch. It was secured with sutures, and the tubing fixed with 3-0 PDS. This approach restored function, improved accessibility, and prevented recurrence.
