Introduction:

Congenital megacorpora is a rare anatomical condition characterized by excessive dilatation and circumferential enlargement of the corpora cavernosa. This results in disproportionate penile girth and contour irregularity, which can cause significant psychological distress due to poor cosmetic appearance and may interfere with sexual function and relationships.

Objectives:

We describe a case that was managed through reduction corporoplasty and the technique involved.

Methods:
A patient with congenital proximal-mid penile megacorpora underwent reduction corporoplasty following multidisciplinary assessment. Preoperative stretched penile length (SPL) was 13 cm, unchanged postoperatively. After penile degloving and circumcision for a constrictive prepuce, artificial erection confirmed markedly dilated corpora from 2–3 cm below the coronal sulcus to the penoscrotal junction, with aneurysmal expansion and attenuated tunica. Targeted tunical plication and reduction were performed bilaterally: on the right, an 7–8 cm elliptical tunical resection with double-breasted 0-PDS closure; on the left, a similar but more proximal 5 cm resection. Repeat artificial erection demonstrated a straight penis with uniform corporal calibre and satisfactory rigidity. Haemostasis was secured and layered closure completed without intraoperative complications. The patient tolerated the procedure well, with no change in penile length and immediate restoration of normal penile contour.

Results:

Reduction corporoplasty was performed bilaterally with tunical excision and double-breasted closure. Postoperative artificial erection demonstrated a straight penis with uniform corporal calibre and satisfactory rigidity. Penile length was preserved, and there were no intraoperative or immediate postoperative complications.

Conclusions:

Reduction corporoplasty represents an effective surgical option for selected patients with congenital macropenis, achieving satisfactory anatomical and functional outcomes.
