Plaque Incision and Grafting With Penile Prosthesis Implantation for Severe Peyronie’s Disease and Erectile Dysfunction: A Case-Based Surgical Approach

Peyronie’s disease with concomitant erectile dysfunction presents a surgical challenge when severe curvature persists despite penile prosthesis implantation. In such cases, additional maneuvers are required to achieve satisfactory straightening. We present a case where plaque incision and grafting was performed in conjunction with inflatable penile prosthesis implantation. After cylinder insertion, marked dorsal curvature remained. The midline penoscrotal incision was extended, the neurovascular bundle carefully elevated, and a transverse plaque incision made using low-energy cautery. A tailored graft was sutured to cover the tunical defect, ensuring preservation of penile dimensions.The patient demonstrated excellent recovery with no complications, straight erection, and functional device performance. This approach highlights the value of combining plaque incision and grafting with inflatable penile prosthesis implantation in selected patients, offering effective correction of deformity and restoration of sexual function with favorable early outcomes.
