Description
Infection is a devastating complication of inflatable penile prosthesis (IPP) procedures, typically necessitating the removal of all device components. This video demonstrates a laparoscopic approach to IPP reservoir removal in a 72-year-old gentleman with an infected IPP. He developed an overt scrotal IPP infection just weeks after a separate procedure to repair a herniated IPP reservoir with an associated posterior inguinal wall weakness, which required an inguinal mesh repair.
Given the integrity of the mesh and the anticipated difficulty of removing the reservoir via a scrotal incision, a laparoscopic-assisted approach was utilized. The IPP cylinders and pump were removed through a scrotal incision, followed by the successful laparoscopic extraction of the reservoir. The patient had an uneventful recovery with plans for reimplantation in six months. This case demonstrates that a laparoscopic-assisted approach is a feasible and safe technique for the removal of a difficult-to-extract IPP reservoir and underscores the importance of pre-operative planning in such complex cases to achieve a favorable outcome.
