This 62 year old patient suffered from a penile fracture 2 years ago. He was a heavy smoker and had coronary stenting after a myocardial infarction one year ago. He developed Peyronies disease with a 45 degree curvature to the left and a hourglass deformity. He was treated with PDE-5 inhibitors and intracvernous injections without success.Pharmaco doppler demonstrated arteriolar insufficiency.
In this video we demonstrate surgical treatment with a Rigicon Infla 10 AX inflatable prosthesis. The surgery is done with a single penoscrotal incision. We demonstrate that thorough dilatation with HL dilators is able to correct curvature and hourglass deformity sufficiently making additional surgical steps unnecessary to tackle the Peyronies deformities. In this video we show furthermore the technique of using a babcock clamp to have full control on the urethra. We also demonstrate our technique of reservoir placement with a medial incision in the fascia of the external oblique muscle. This is an incision medial to the external inguinal orifice. This incision in the fascia is then closed with a preplaced “W” vicryl 1 suture. By doing this we prevent future herniation of the reservoir. Furthermore we want to draw attention to the technique to prevent cross-over by inserting a HL dilator on the contralateral side while dilating the corporal cavity and inserting the cylinders.
