Description

In this video we operate on a 54 year old male patient. Seven years ago he underwent a total coloproctectomy for an adenocarcinoma of the rectum stage pT3. The patient received an ileo-anal pouch.The patient was non responding to conservative therapy with PDE-5 inhibitors and intracavernous injection therapy. Technical exam showed on a pharmaco doppler an arteriolar insufficiency.

The video shows a penile prosthesis implantation through a single penoscrotal incision. It is demonstrated that even in a hostile pelvis reservoir placement can be executed with a single incision even with full visual control. To do so perfect control of the urethra is necessary by using the Babcock clamp. With proper retractors a clear view on the fascia of the external oblique muscle is possible. We establish an incision medial to the external inguinal orifice. This incision in the fascia is then closed with a preplaced “W” vicryl 1 suture. By doing this we prevent future herniation of the reservoir. We also demonstrate how important proximal corporotomies are to prevent using rear tip extenders as much as possible.
