Description

Reservoir placement in inflatable penile prosthesis (IPP) surgery is often complicated in patients with prior extensive abdominal or pelvic surgeries. The traditional retropubic approach may pose risks of bowel or vascular injury in such cases. This video presents the high sub-rectus space as a viable alternative for reservoir placement, demonstrated in a patient with complex pelvic trauma and multiple prior abdominal interventions. The reservoir was positioned between the rectus abdominis muscle and posterior rectus sheath. The procedure was completed successfully with no intraoperative complications or postoperative issues, and the patient was discharged the following day. This approach is particularly beneficial when the retropubic space or inguinal rings are inaccessible. High sub-rectus placement provides a safe, effective solution in anatomically challenging scenarios and should be considered in the surgical planning for select IPP candidates.
