Description

Robotic-assisted Burch colposuspension using the da Vinci system offers enhanced precision for the surgical management of stress urinary incontinence. This video covers the key anatomical identification urethrovesical junction, urethra, and paravaginal fascia and different surgical technique of anchoring the paravaginal fascia to Cooper’s (pectineal) ligament.

After standard port placement and docking of the da Vinci robotic system, the

retropubic space (Retzius space) is carefully developed. The urethrovesical junction and urethra are clearly identified as key landmarks for optimal suture placement. The paravaginal fascia is then dissected bilaterally to allow adequate mobilization while preserving surrounding neurovascular structures and avoiding bladder and urethral injury. Suspension sutures are placed through the paravaginal fascia on each side

and securely anchored to Cooper’s ligament, providing elevation and stabilization of the urethrovesical junction. This technique restores the anterior vaginal wall support and improves urethral continence mechanism without tension on the urethra.
