Description

This video presents a challenging case of distal penile prosthesis erosion after multiple revisions in a 66-year-old male. The patient, an active smoker, underwent several previous surgeries in another center: initial implantation of an AMS 700 inflatable prosthesis (May 2022), explantation and replacement with a TACTRA malleable prosthesis (May 2023), downsizing revision (October 2023), and left cylinder removal for impending extrusion (May 2024). He was referred to our center for right-sided extrusion and requested reconstruction with a three-component hydraulic prosthesis. The video highlights the surgical strategy for reimplantation in a scarred corporal environment, including fibrosis management and cylinder placement. This complex case underlines the importance of meticulous planning, technical expertise, and patient-centered decision-making in revision penile prosthesis surgery.
