Combined Use of Transverse Ventral Tunical Incisions and Tachosil Grafting During Penile Prosthesis Implantation in Severe Ventral Penile Curvature

Introduction:

Congenital ventral penile curvature associated with erectile dysfunction can persist following inflatable penile prosthesis (IPP) insertion, occasionally requiring additional corrective straightening maneuvers.

Objectives:

To describe a surgical technique of multiple tunical incisions with TachoSil® grafting for correction of residual ventral curvature following IPP placement.

Methods:

A patient with congenital 90° ventral curvature and erectile dysfunction underwent IPP implantation. Despite optimal cylinder positioning, a residual 50–60° ventral curvature persisted. The urethra was carefully elevated, and multiple transverse tunical relaxing incisions were made along the ventral aspect over the inflated cylinders. The tunical defects were then covered using a TachoSil® graft.

Results:

Complete penile straightening was achieved intraoperatively with preservation of urethral integrity and adequate cylinder expansion. No intraoperative complications occurred.

Conclusions:

Multiple tunical incisions with TachoSil® grafting is a safe and effective adjunctive technique to correct significant residual ventral curvature after IPP insertion, offering functional and cosmetic restoration in select complex cases.
