Description
Introduction:
Penile prosthesis implantation is an effective treatment for erectile dysfunction but may be complicated by infection, erosion, and corporal tissue loss.
Case Presentation: We report a 64-year-old man with a history of malleable penile prosthesis insertion complicated by urethrocutaneous fistula, infection, and tissue necrosis. After device explantation and debridement, he presented with a large 4.5-cm corporal defect. Reconstruction was achieved using a vascularized tunica vaginalis flap and implantation of an 18-cm Rigicon malleable penile prosthesis. The flap was reinforced with TachoSil® collagen fleece. Postoperative recovery was uneventful, with satisfactory device function and no recurrence of infection.
Discussion:
Corporoplasty in revision penile prosthesis surgery is technically challenging. Autologous tunica vaginalis flap offers vascularized tissue coverage with low risk of infection and no additional donor site morbidity. This technique is particularly valuable in resource-limited settings where commercial grafts are not available.
Conclusion:
Tunica vaginalis flap provides a practical, biologically sound, and cost-effective reconstructive option in complex penile prosthesis revision cases.
