This video presents two cases of retained component retrieval during penile prosthesis revision surgery. In the first case, a rear tip extender was identified to be detached preoperatively via CT imaging and retrieved during corporal exploration. The second case involves intraoperative identification and removal of a metallic tubing cap aided by intraoperative radiography. Retained components such as rear tip extenders and metallic connectors, though uncommon, pose significant clinical risks. This video emphasizes diagnostic vigilance in revision settings, highlights the role of imaging in localizing retained components, and demonstrates retrieval techniques. The case series underscores the importance of implementing a structured surgical checklist to ensure complete component removal during prosthesis explantation and prevent avoidable complications.
